Terms and Conditions

Welcome to Momentum Disability Services. These Terms and Conditions govern the provision of disability and aged care support services to our valued participants and their families in accordance with Australian law and NDIS guidelines.

1. Service Provision

Momentum Disability Services agrees to provide supports to the Participant as outlined in their individual Service Agreement. We are committed to delivering person-centred care that empowers independence and respects the choices of our clients. Services are reviewed regularly to ensure they meet the evolving needs and goals of the individual.

2. Rights and Responsibilities

Participants have the right to be treated with dignity, respect, and to be free from discrimination. It is the responsibility of the Participant or their representatives to provide accurate information to assist in service planning and to treat our staff with courtesy and safety while they are providing care in the home or community.

3. Fees and Payments

Service fees are set in accordance with the NDIS Price Guide where applicable. Invoices will be issued fortnightly and are payable within 7 business days. For participants who are agency-managed, we will claim directly through the NDIS portal. Self-managed participants are responsible for ensuring funds are available for payment of services rendered.

4. Cancellation Policy

We understand that schedules can change. To avoid short-notice cancellation fees, please provide at least 24 hours' notice for any service cancellations. Cancellations made with less than 24 hours' notice may be charged at 100% of the scheduled service fee in line with NDIS guidelines to protect our support workers' livelihoods.

5. Privacy and Confidentiality

Momentum Disability Services is committed to protecting your privacy in accordance with the Privacy Act 1988 (Cth). We only collect information necessary to provide safe and effective care. Your personal details will not be shared with third parties without your explicit consent, unless required by law or in emergency situations.
